How to Apply
Students from a Guthrie-affiliated advanced practice provider program: Rotations for advanced practice provider students from programs affiliated with Guthrie Robert Packer Hospital are scheduled in advance through the individual schools.
Schools must provide:
- Visiting Student Rotation Request Form
- Clearance documentation of criminal and child abuse background checks (Child Line Application)
- Proof of immunization records
Students from an unaffiliated advanced practice provider program: Guthrie requires an active affiliation for all students. For more information on how your institution can apply to become an affiliated program, please contact the Student Coordinator at 570-887-6583.
