Guthrie Medical Group Received Accreditation

Guthrie Medical Group’s Primary Care locations celebrated the annual accreditation for Patient Centered Medical Home through the National Committee for Quality Assurance.

Patient-centered medical homes (PCMH) are a care model for primary care practices that provides health care that focuses on patients and their needs. Under this care model, everyone in the practice, from providers to front desk staff, works as a team to coordinate care from other providers and community resources. PCMH helps patients become engaged in their own healthy behaviors and health care. Medical homes are the foundation for a health care system that achieves the “Triple Aim” of better quality, experience, and cost.

To earn reaccreditation, a practice must complete criteria in each of the following concept areas: team-based care and practice organization, knowing and managing patients, patient-centered access and continuity, care management and support, care coordination and care transitions, and performance measurement and quality improvement.
