Home Phlebotomy Services:

To better meet the needs of our patients, Guthrie Cortland Laboratory provides Home Phlebotomy Services. Patients who are unable to travel and meet the Medicare Homebound guidelines, may have blood collected in their home by one of Guthrie’s phlebotomists. A physician order for homebound phlebotomy is required.

Patient Eligibility:

  • Medicare provides a definition of "homebound" for eligibility.
    • A person does not have to be bedridden to be considered confined to the home.
    • The condition of the patient should be such that there is a normal inability to leave the home.
    • Leaving the home would require a considerable and taxing effort.
    • Absences from home are infrequent and are usually to receive medical care.
  • Reside within Cortland County
  • If two attempts are made and the patient is not available - services will be discontinued.

Provider Requirements:

  • Guthrie Providers: Complete a referral for home draw in Epic.
  • Non-Guthrie Providers: Call the laboratory to arrange for homebound patient phlebotomy.
    • Ensure all required patient demographic, patient diagnosis, patient test information, special instructions, insurance card copies and medical necessity information are included on and with the laboratory requisition. Missing information will cause a delay in scheduling this service.
    • Fax completed laboratory requisitions to the Laboratory. Faxed requisitions are scanned into the patient's EMR.
  • Must notify laboratory if patient becomes ineligible.

Limitations/Scheduling:

  • Services are scheduled by laboratory based on our availability.
  • Services are not available on weekends or Guthrie recognized holidays.
  • Request services at least one week in advance.
  • Requests received with less notice will be scheduled as soon as possible.
  • Laboratory may not be able to accommodate a specific arrival time.
  • STAT services are not available.

  • Laboratory tests fees
  • Phlebotomy service charge
  • Milage fee – calculated rate on distance
