  • Improve physician knowledge to facilitate complete and accurate documentation that is compliant under the ICD-10CM requirements.
  • Identify documentation practices that support accurate capture of severity of illness (SOI) and the Risk of Mortality (ROM).
  • Describe how documentation impacts patient care and quality reports for both the individual physician and the hospital.
