Sayre, Pa. – Nominations are now being accepted for the Donald Guthrie Distinguished Provider Award for providers within the Guthrie system.
Each year, the Guthrie Board of Directors and Reward and Recognition Committee of the Guthrie Clinic seek to recognize providers who epitomize the mission statement of Guthrie and the ideals of Donald Guthrie, MD. Those eligible for nomination include physicians, physician assistants, nurse practitioners, midwives, optometrists, physical therapists, dieticians or other clinicians.
A provider can be nominated by another provider, an employee, or a patient or patient’s family. Nomination forms are available in the Guthrie Clinic atrium and Clinic administration offices in Sayre, Pa., in waiting rooms in the Guthrie regional offices throughout the Twin Tiers and on www.guthrie.org.
Completed nomination forms can be sent to: Reward & Recognition Committee, Attn. Melanni Winslow, Guthrie Clinic, One Guthrie Sq., Sayre PA 18840 by September 28, 2012 or e-mail your story to firstname.lastname@example.org. Drop off boxes for completed nominations are also in the clinic reception areas, regional offices or on the desk in the atrium lobby.
Nominations are due by Sept. 28.
