Osteoporosis Risk Assessment

The first step in caring for yourself  is knowing if you may be at increased risk for osteoporosis. While the questions below are no substitute for a regular check-up with your family physician, it can help to have some idea where you stand in terms of risks for osteoporosis. Good luck! If you do not have a physician and you would like to find out more about osteoporosis, call toll free at  1-888-4GUTHRIE (1-888-448-8474).

Evaluation:

If you are between 50 and 64 years old and you have additional risk factors, (especially any one of those that are printed in teal), you should probably undergo a bone density study to determine if you do have osteoporosis. Your Guthrie Primary Care Provider can refer you for this study.

If you are a woman 65 years of age or older should have a bone density study to determine whether osteoporosis is present according to the National Osteoporosis Foundation.

If you are less than 50 but have many of these identified risk factors, (particularly more than one of those that are printed in teal), you should discuss the issue of osteoporosis with your family physician.

Please select "PRINT" on the File menu to print this form to discuss it with your Guthrie Physician.
