What does the HomMed® system do?

The HomMed® system can provide you with fast, accurate and simple access to daily vital sign monitoring such as heart rate, blood pressure, blood oxygen saturation, weight and temperature. Lung function and blood glucose (sugar) levels will be available in the future. The HomMed® system allows you to actively participate in your own health care by providing state-of-the-art, daily screening, regardless of how far away your home is or what the weather is like.

How does HomMed® work?

The HomMed® unit, which takes up less space than a computer, is easily set up by a Guthrie Home Care clinician in your home. All that is needed is a standard electrical outlet and telephone jack. The HomMed® system is then programmed with whatever settings and health questions your doctor recommends.

At the same time each day, a voice prompt will instruct you to take your vital signs. The voice guides you step-by-step through the process of pushing one of three color-coded buttons that enable the system to collect complete, clinically accurate vital sign data. Then the system will ask you to answer a series of questions that your doctor has provided. It takes only 3 minutes a day and it is easy to do!

Once you have completed the process, the HomMed® system transmits the data to a clinician at Guthrie Home Care who reviews the data. If there is anything out of the ordinary in the readings, the clinician will call you. If any vital signs fall outside the readings your doctor recommended, the unit will signal our clinicians and alert them of your condition. The problem can then be identified and treated before it becomes serious.

Your Guthrie Home Care team will keep track of your readings and provide your doctor with weekly or monthly graphs of your vital signs. These reports allow your doctor to see subtle changes in your condition and schedule your follow-up office visits accordingly. These reports also help your doctor to adjust your treatment when needed. Patients who use the HomMed ®system often have fewer visits to the emergency room and to their doctor’s office.

Even if you’re not currently under a physician’s care, the HomMed® system can provide you and your family with the sense of security that comes from knowing your health is being monitored on a daily basis.

The HomMed® system compliments your other home health care & monitoring

The HomMed® system is not meant to replace, but rather to complement, visits by your Guthrie Home Care professional. The HomMed® system allows them to monitor how you’ve been doing on a daily basis even if they aren’t visiting your home every day.

The HomMed® system is not a substitute or replacement for Lifeline®, 911 or other emergency systems. If an emergency arises, the HomMed® system cannot signal paramedics, firefighters or police for immediate help.

Medicare, Pennsylvania Medical Assistance, Blue Cross or major medical coverage and other private insurance cover most Guthrie Home Care services. However, lack of coverage or inability to pay for services does not mean loss of service. We will review your insurance coverage and explore alternate funding.
