SUBMISSION INFORMATION AND FORM Samples should be sent to:

Donald Guthrie Foundation for Education & Research Inc Submission form #1 provides contact and billing information. Submission form #2 provides sample descriptions and tests requested. You will need to complete both submission forms and submit them electronically. Please include a paper copy with your samples. * Please do not send credit card information electronically at this time as it is not secure to to send this via email or fax. This will need to be given on the paper copy submitted with the samples. Samples should be placed in individual bags (original wrapper if possible) to prevent cross contamination during shipping. Samples submitted for testing should be clearly labeled. Samples are identified in the reports as you have labeled them. Being more descriptive will make it easier to keep track of your samples. For example, if you are submitting a glove to be tested you can label it simply "Sample A" or you could be more descriptive, "Non-powdered Exam Glove Lot #123456." NOTE: Once a report has been compiled, we will not change the sample description unless it is to correct a typographical error. Be sure to include information on which test or tests you would like performed on your samples. For the ASTM D5712-05 assay, three items are needed for each sample. If possible send additional test items. The different protein assays employ different extraction procedures. Thus it may be necessary to send multiple items for multiple tests. In the event we need to repeat an assay on a particular test item, we can proceed without having to wait several days for the additional time of shipping.
