Sayre House Care Plans

A care plan for every resident In a home-like setting at Sayre House, residents receive a complete range of rehabilitative, medical, nursing, nutritional, recreational and social services. Using a team approach, and involving family and friends, a personalized care plan is developed for all residents that address their physical, emotional, and social needs.  

Therapists can provide a variety of rehabilitative options to help residents attain highest functional capability.  Restorative nursing programs enable residents to maintain these capabilities.  Our staff is trained to care for the needs of residents with dementia and Alzheimer's Disease by providing specially designed activities and latest treatment programs.  Our medical director is a physician member of Guthrie's medical staff.

Our Social Services department helps residents to make the change to their new residence.  Staff members also counsel residents and their families on legal, financial and ethical issues, as well as help with discharge planning.  If a resident needs another level of care, the resident can move easily and seamlessly within the Guthrie system to home care or hospital care.
